Seeking a gender-affirming surgery letter shouldn't feel like another barrier to receiving the care you deserve.
The Accompli-Cis offers pro bono, one-session surgery letter assessments for transgender, non-binary, and gender-expansive adults through secure video telehealth. The goal is to make this part of your journey feel supportive, collaborative, and straightforward.
Whether you're preparing for top surgery, bottom surgery, facial feminization surgery (FFS), body contouring, or another gender-affirming procedure, you will be met with respect, curiosity, and affirming care.
What You Can Expect
Appointments are held via secure video telehealth and are typically scheduled for 60 minutes, with additional time available when clinically appropriate. Sessions are currently offered on Fridays.
Following your assessment, if the WPATH Standards of Care criteria are met, your surgery letter will be completed in accordance with current professional guidelines and the documentation commonly requested by surgeons and insurance companies.
This service is offered completely free of charge (pro bono). Because there is no cost, insurance information is not required.
About Your Provider
Surgery letter assessments are provided by a white, queer, cisgender woman therapist with significant experience writing gender-affirming surgery letters.
Letters have been written in support of individuals pursuing:
Top surgery
Bottom surgery
Facial Feminization Surgery (FFS)
Body contouring procedures
Other gender-affirming surgical care
In addition to providing surgery letters, The Accompli-Cis can also offer advocacy when needed. This may include communicating or collaborating with healthcare providers, surgeons, or other members of your care team to help support timely access to gender-affirming care.

Who This Service Is For
At this time, surgery letter assessments are available for adults age 18 and older. This reflects the clinician's current scope of practice, training, and experience.

The clinician will determine whether services can be provided by seeking appropriate legal consultation regarding telehealth and documentation requirements. If services cannot be provided in your state, referrals to affirming providers and resources will be offered whenever possible.
